Most of the time you can’t "afford" to have such insurance if something does happen. Let’s say, in California minimum liability is $15,000 per person which will pay for the ambulance to get to the scene and after that you are on your own. If you have no money than get a policy that you can pay for and drive legally. And it also depends on if you have medical insurance or not. Probably it would be best if you contact a local insurance agent to get familiar with options and benefits in respect to your situation.
